Adult Day Program Services

What is the Adult Day Program?
It is a unique program for older adults who are either physically frail or cognitively impaired.  This “home away from home” focuses on the promotion of independence, well-being and friendships.  Transportation and meals are provided.  The program also addresses special dietary requirements and needs.

Where are we Located?
The program is located at 840 Coxwell Avenue, Suite 101 (on the ground floor of the building directly across from Toronto East General Hospital)

Who Can Attend?
• residents of East York
• those who are socially isolated or living at risk within the community
• those who need assistance with activities of daily living
• those whose caregiver(s) require respite
• those with lack of transportation to go out and socialize
• those who need opportunities to meet new people and socialize

Program Highlights
• The program runs daily from 10:00 a.m. to 3:00 p.m.
• Transportation will be provided to and from, when needed.
• Clients will be provided with hot lunches and snacks.
• Recreational activities reflect client interests.
• Space is bright, level, wheelchair accessible and secure.
• Friendly, caring staff offer one-on-one assistance, if needed.
• Programs are client-centred, with no more than eight participants.
• Fee for the program is $20.00 with transportation and $8.00 without.
• Subsidy available upon financial assessment.
• Escorted transportation is available for clients who require assistance door to door.

The Adult Day Program will provide opportunities for:
