Director of Client Care Services

We are looking for a highly motivated, results-oriented leader to join our senior management team in working toward implementing the strategic directions of the agency and developing the opportunities that exist in the broader health and social service sector.

Reporting to the executive Director, your primary responsibilities will include:
• Business development
• Developing and working with program managers to ensure that services are delivered in a manner that aligned with the organization's strategic prioities and consistent with the agency's philosophy, mission and values. 
• Using a systems approach to ensure best practices
• Providing leadership and direction to program staff to enable the programs to achieve their goals and objectives. 
• As a member of the senior management team you will also share the responsibility for agency-wide initiatives and ensuring that the agency is functioning effectively.

Qualifications:

• A degree in Health Administration or equivalent education and experience
• Three to five years of experience of management experience in a community health setting
• Proven business  development experience
• Experience in nurturing and building external partner relationships
• Proven leadership skills
• Excellent interpersonal and organizational skills

Psycho-Geriatric Case Manager

This Community based Psycho-Geriatric Program, a partnership between Toronto East General Hospital, Wood Green Community Centre and Community Care East York is seeking an experienced enthusiastic individual to work  a flexible scheduled 28 hours a week.

Nature and Scope:
To work as a member of an intensive case management team to assist older persons (age 60 plus) coping with severe mental illness, cognitive deficits or changes in behaviour related to the aging process.  The incumbent also provides consultation to other professionals and peers around complex mental health situations.

Qualifications:
• MSW or equivalent training/experience
• 2-4 years experience in psycho-geriatrics/mental health
• Good understanding of case management and PSR principles
• Sound clinical skills
• Excellent communication and interpersonal skills
• Ability to work as an effective team member and independently in the community
• Knowledge of East Toronto and mental health community resources an asset
• Must speak a second language- Gujarati, Urdu or Greek

Social Worker

We require an experienced, enthusiastic social worker who has skills working with a range of client populations including frail, cognitively impaired adults and their caregivers.

RESPONSIBILITIES:

Counselling/Case Management:  assessment, referral, service co-ordination, crisis intervention, advocacy, counselling, case management, outreach and monitoring.
Intake Services: telephone intake, screening and assessment, information, consultation, referral and follow-up.
Community Outreach: provide information about agency services
Group Work: caregiver education and support group, bereavement group, in-service education.

QUALIFICATIONS:
• Masters of Social Work (or equivalent education/experience), eligibility for membership with OCSWSSW.
• Experience in working with older adults, adults with disabilities, adult caregivers,     individuals and families.
• Strong assessment, relationship building and counselling skills.
• Excellent interpersonal, problem solving and organizational skills.
• Ability to work independently and collaboratively within a team, and in the community.
• Ability to effectively manage the demands of a high volume of complex cases.
• Experience conducting home visits within the community.
• A car and a valid driver's license an asset.
• A second language is an asset.

Women’s Support Network Coordinator

We require an experienced, enthusiastic coordinator who is able to engage women in the development and maintenance of an ongoing older women’s support network. 

RESPONSIBILITIES:
β€’ Conduct focus groups.
β€’ Plan, promote and facilitate older women’s support groups.
β€’ Organize educational sessions focusing on social economic issues of aging.
β€’ Create an older women’s network to develop mutual support through the transitional phases of aging.
β€’ Develop capacity of older women in the areas of finance, health, self advocacy, and community engagement. 
β€’ Design and conduct evaluation to ensure program objectives are achieved.

QUALIFICATIONS:
β€’ Excellent communication, networking and interpersonal skills.
β€’ Strong group development and facilitation skills.
β€’ Ability to mobilize women to engage in the support network, support groups, and educational sessions.

Driver Program Assistant

We are seeking a part time assistant/driver for the CCEY transportation Program.

POSITION SUMMARY: 
To provide safe and reliable transportation to seniors and adults with disabilities for medical/therapy appointments

MAJOR RESPONSIBILITIES:
• Drive agency vehicles to transport clients as directed by Transportation Coordinator
• Assist clients in and out of vehicle when necessary and folding assistive devices
• Ensure that clients fasten their seatbelts and assist clients when necessary
• Daily Vehicle Maintenance Check
• Ensure vehicle is properly secured at the end of each shift

QUALIFICATIONS:
• Valid Class G driver's license
• Personal Drivers Insurance
• Must be 25 years of age or older
• Clean and recent driver's abstract
• Excellent communication and interpersonal skills
• Ability to work independently and part of team
• Knowledge of East York area
• Sensitive to the needs of seniors and individuals with disabilities

Community Care Workers/PSW's

Community Care East York offers a variety of health and support services to the elderly in the East York Community.   We are looking for Community Care Workers to provide personal care services to the elderly and people with disabilities in a Supportive Housing setting over the coming holidays (December to January) with the possibility of ongoing employment. 

GENERAL DUTIES:
• Provide personal care for clients in their homes
• Provide light housekeeping for clients in their homes
• Complete the client progress forms to update staff about client changes

QUALIFICATIONS:
• Personal Support Worker Certificate or R.N./R.P.N. student
• 1 year of experience in a community setting
• Strong communication and written skills
• Demonstrated ability to work both independently and within a team
• Physically able to support clients during transfers from position to position
• Valid driver license and car available, an asset
• Culturally sensitive
• A second language would be an asset

Summer Students

Are you looking for experience in a non-profit community agency?  Would you like the opportunity to work within a diverse multi-disciplinary team and learn new skills?

Community Care East York particpates in the Canada Jobs Summer Jobs Program.  This year we are looking for 2 students to work in:

Adult Day Program team:

Duties include assisting  the team with the development and implementation of social recreational programs to frail and cognitively impaired seniors and peiople with disabilities.

Social Work/In-Home Services team. 

Duties include providing a variety of supports to both teams including answering and responding to inquiries, scheduling, filing, contacting clients. 

Qualifications:

  • Enrolled in post secondary program in a related field i.e. Social Service, Genretology/Social Work - an asset
  • Excellent communication skills
  • Good problem- solving skills
  • Good computer skills

Students who are eligible to participate must:

  • be between 15 and 30 years of age;
  • have been registered as a full-time student in the previous academic year, and intend to return to school on a full-time basis in the next academic year;
  • be Canadian citizens, permanent residents, or have official refugee protection status under the Immigration and Refugee Protection Act; and
  • be legally entitled to work in Canada, according to relevant provincial/territorial legislation and regulations
