Community Care East York seeks through our, Integrated Wellness Project Healthy Futures to build community capacity that increases access to and participation in CCEY’s enhanced Wellness Programs. Our programs are designed to fulfill the unmet needs of people with disabilities and seniors living in East York, ensuring they have the opportunity to reach their full health and well-being potential.
