The goal of the Supportive Housing Program is to ensure the independence, safety and well being of residents.
Support is offered to eligible seniors and adults with disabilities at three Toronto Community Housing buildings, (12 Thorncliffe Park Drive, 266 Donlands
Avenue and 9 Haldon Avenue) and in a residential home (O'Connor House). O'Connor House is designed to accommodate 10 frail seniors who need assistance with activities of daily living.
Services include on site 24 hour assistance. Trained staff assist residents with personal care, meal preparation, medication reminders, light housekeeping and safety checks.
What is Supportive Housing?
Supportive Housing provides personal care and housekeeping to tenants in the following three Toronto Housing Company buildings:

The services are provided to those people who need help in order to live independently in the community. Services are available 24-hours a day and on an emergency basis.

What is Emergency Service?
Staff from Community Care East York are in the building around the clock, seven days a week and can be reached through a pager. A pager is linked to an Emergency Response System - Cardio Link - so that residents who have the system may access help if they have fallen and are not able to reach their telephone. This service is available to all of our Supportive Housing clients.
Hours: Supportive Housing services are scheduled in small blocks of time, are
individualized and available 24 hours/day, 7 days/week.
Costs: Supportive Housing is funded through the Ministry of Health and there is no charge for personal care and essential housekeeping services for clients of Supportive Housing.
